Required Medical Information + Health Forms (By Neilia Bliss, School Nurse and Anatomy & Physiology Instructor)

We're excited for the coming year and all the athletics, fine arts and academic TAPPS competitions. The following contains important information from the health office and athletics office regarding required health forms. PLEASE NOTE: THIS IMMEDIATELY IMPACTS ANY STUDENT PLAYING A FALL SPORT. Students will be ineligible to participate until they comply with this requirement. 
 
For the 2021-2022 school year, there are two steps to completing required health documentation.
  1. The health office requires completion of student medical information in Magnus, our electronic health records system.
  2. TAPPS, the Texas Association of Private and Parochial Schools, requires each student has an account and current physical on file in the Rank One electronic system. 
New and returning families can start completing required information for Magnus.
  • Magnus health information is found at mySMCA.  Click HERE to log into mySMCA.
  • Log in, click on the RESOURCES tab at the top of the page, and go to the Magnus Health Records tile.  In Magnus, complete all items marked “To Do.”
    GREAT NEWS!  Magnus now also has an app where you can easily complete, scan and upload all of your student’s requirements.  Please refer to this document for info!
  • New families must provide current immunization records and screenings (documentation from middle school can satisfy screening requirements, please try to contact your middle school now to retrieve screening documentation).
Returning families will need to update all checklist items.
  • After updating, items will be marked “Pending School Approval”; after approval from the health office, items will be updated through August 1, 2021. Please email the nurse, Neilia Bliss, RN, MA, at nbliss@smca.com with any questions.
New and returning families can start completing required information for Rank One on June 1.
  • To create or complete your student’s Rank One account, click here.
  • Follow the steps for parents to the online forms for St. Michael’s Catholic Academy.
  • ALL TAPPS parents are required to create an account to be able to access the online forms and will need to use their child's student ID to do so.  Student IDs may be found by going to the student profile and selecting the student Contact Card and then General Information.
  • Documentation of a current physical must be uploaded into your student’s Rank One account.  Current physical (physicals are only good for 1 year from the date performed.)  A blank physical form can be found here.  Please email me at jdicus@smca.com with any questions.
