Health Forms in Rank One Sport (By Caitlin Fegley, Associate Athletic Director)

The following contains important information from the athletic office regarding required health forms. Students will be ineligible to participate in sporting events until they comply with this requirement.
 
TAPPS, the Texas Association of Private and Parochial Schools, requires each student to have an account and current medical forms on file in the Rank One electronic system. New and returning families should complete the required information for Rank One as soon as possible.

FOR ALL NEW/TRANSFER ATHLETES:
  • To create your athlete's Rank One account, click here. You will use this same account login throughout your time at St. Michael's.
  • You'll need your child's student ID number to attribute your new account to your athlete. Student ID numbers may be found in their contact card in MySMCA.
  • We recommend completing the "TAPPS Student Profile" before moving on to other documents needed. Pay special attention to your answers to all of the questions asked.
  • Documentation of a current physical evaluation must be uploaded into your student’s Rank One account, along with the other required forms listed in your portal. Physicals are only good for 1 year from the date performed. A blank physical form can be found here. Please note, this is NOT the same form that is submitted to Nurse Neilia Bliss.
  • Please upload your athlete's physical to Rank One, and complete all other online forms needed.

FOR ALL RETURNING ATHLETES:
  • If your athlete played a sport at St. Michael's in the past, you should have an existing account with Rank One. To log in to an existing account, click here.
  • Please fill out all forms required in your athlete's profile. We recommend completing the "TAPPS Student Profile" before moving on to other documents needed. Pay special attention to your answers to all of the questions asked.
  • Documentation of a current physical evaluation must be uploaded into your student’s Rank One account, along with the other required forms listed in your portal.
